Dr. Hoang and his associates take their time with each patient to ensure you get the time and care you deserve. If a treatment is recommended, we will take the time to explain and go over options and answer any questions or concerns you may have. Thank you for taking time to visit our website and for downloading our new patient packet. In order for your appointment to begin on time, please review the following checklist and bring each of the items listed on it with you to your appointment. If you need directions to our office, you can visit our website or call us directly. We are happy to help!
✔ Picture ID (driver’s license or other government issued identification card with photograph).
✔ Dental Insurance Card (without this card, we will not be able to file your insurance claim).
✔ Completed New Patient Registration Form (please file out ALL applicable portions including social number and date of birth).
✔ Completed and Signed Medical History Form (please be thorough).
✔ Signed Office Policy form (if minor, signature needs to be by the person who is financially responsible for the patient).
✔ Signed General Consent form
✔ Signed No Show/Late Cancellation Policy
✔ Signed Email & SMS Communication Release
✔ Signed Photography Release
✔ Keep a copy of HIPAA form for your record
✔ A form of payment (we accept all major credit cards as well as personal checks and cash).

If you find that you cannot arrive for your appointment on time, please make sure to give our office at least 24 hour notice.
Insurance Carriers
Nearly all major insurance plans accepted
If you have dental insurance, we will be happy to help you maximize your benefits. Please bring your ID card and insurance card with you to each visit so that we can make sure our records stay current. As a courtesy to you, we will file and track all claims and provide postage for special claims. Please note that your insurance policy is a contract between your employer, your insurance company and you, and that you are responsible for the payment of all professional fees, regardless of the extent of insurance coverage.
In-Office Discount Plan
Our goal is to help you make dental visits more affordable so you can get the quality dental care you want and need. Our very own in-house dental discount plan will provide you with tremendous savings. This reduced-fee dental plan allows individuals or families to receive all the available dental services at Geaux Smiles for a percentage discount of the regular office fees. If you have recently lost your dental coverage or have never had dental coverage, this membership may be right for you.
